A nurse visits your home to assess your loved one's needs, answer questions, and discuss care options. This visit is free and helps us understand how we can best support you and your family.

Care Plan Development

Our team works with you, your loved one, and their physician to create a personalized care plan. We'll discuss goals, preferences, and any specific needs or concerns. Your input shapes everything we do.

Admission & Care Begins

Once enrolled, care begins immediately. We'll deliver any needed medical equipment and supplies. Your care team introduces themselves, and regular visits are scheduled. You'll have 24/7 access to our on-call team.

Levels of Care

Wherever your loved one calls home, Affinity Healthcare is committed to providing quality care. Medicare-certified hospice providers offer four levels of care, each tailored to the patient’s unique needs.

Routine Home Care

The most common level of care. Our team provides regular visits while you remain comfortable at home, surrounded by loved ones and familiar surroundings.

Respite Care

Short-term inpatient care (up to 5 days) in a skilled nursing facility to give family caregivers a needed break while ensuring continuous quality care for your loved one.

Continuous Home Care

Intensive, short-term care at home during a medical crisis or acute symptom management needs, with nursing staff available for extended periods.

General Inpatient Care

Hospital or facility-based care when symptoms become too complex to manage at home, requiring 24-hour skilled nursing supervision and support.
