Affinity Hospice is hiring for a On-call PRN RN Case Manager to join our family of employees in the Lufkin, TX area. The ideal candidate will have: at least 2 years of hospice care experience, excellent time management skills, attention to detail, ability to effectively and efficiently document care and communication, and most importantly the desire to provide compassionate care to our mostly geriatric patient population. Travel is required but a fleet vehicle may be available as needed.
