Job Description

Job Summary:Affinity Healthcare is hiring for a dedicated and compassionate Clinical Nurse Manager to join our Home Health team. The ideal candidate will be able to provide oversight of all patient/client care services and personnel. Ensure quality of patient/client care in the home. Coordinate care with the interdisciplinary team, patients/clients, family, and referring agency(ies). This role requires a strong understanding of state and federal regulations, medical terminology, home health care, palliative care, and hospice care, along with the ability to efficiently and effectively lead and manage Home Health clinicians. Travel is required and a Company Car will be made available to you. Duties:

  • Coordinates patient/client care. Coordinates and integrates care provided by Agency staff, including contractual providers.
  • Ensures communication with all physicians involved in a patient’s/client’s plan of care and other healthcare practitioners.
  • Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.
  • Participates in activities relevant to professional services. Recommends resources needed to provide care. Makes patient/client and personnel assignments.
  • Assures that patient/client needs are continually assessed.
  • Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient/client, representative (if any), and caregiver(s).
  • Assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations
  • Collaborates on development and implementation of policies and procedures which guide and support the provision of care.
  • Monitors systems and identifies problem areas to Administrator both verbally and through written reports.
  • Ensures the care offered by the Agency, including any branches, is consistently available to all patients/clients.

Experience :

  • Minimum 2 years of Home Health nursing experience required
  • Minimum 1 year clinical leadership/management experience required

Education:

  • Must be a graduate from an accredited associate or baccalaureate nursing program.
  • Must complete Texas HHSC’s Computer Based Training for appropriate category(ies) of license.
  • Bachelor of Science in Nursing (BSN) preferred.
